What is B-Form?

Who can apply for B-Form?

• Parents with Pakistani citizenship
• Who have a valid NADRA National Identity Card (CNIC) or NICOP
• Whose child’s birth certificate has been issued by the Union Council

Documents required for B-Form

• Child’s birth certificate (issued by the Union Council)
• Original National Identity Card (CNIC or NICOP) of the father or mother
• Child’s photograph (for online upload)
• Digital signature of the father/mother (for online upload)

How to apply online for B-Form

If the child is less than one year old, the application can be made online in the following way:

• Log in to PakID mobile app or website: https://id.nadra.gov.pk
• Go to “Issuance of Identity Document”
• Select “Child Registration Certificate (CRC)”
• Enter the 13-digit ID number and start the application process
• Review the pre-filled information
• Upload the child’s photograph and signature Do
• Verify fingerprints of parents
• Submit application

A visit to NADRA office will be required for children above one year of age.

According to NADRA, there has been no increase in B-Form fee for 2025. Processing is completed in a few days through Executive Service.
